Job summary

Elara Caring Hartford is seeking a Licensed Practical Nurse Behavioral Health for PRN shifts in the Hartford, CT area. You will verify plans of care, deliver nursing visits, and support patients at home rather than in a facility.

Qualifications

• Graduate of an accredited nursing program (certificate, diploma, or associate).
• Current State LVN/LPN license in good standing.
• Minimum 1 year of clinical care experience.
• Hospice or home health experience preferred.
• Willingness to travel up to 50% and reliable transportation.

Responsibilities

• Verifies the Plan of Care prior to each visit and provides care per RN delegation.
• Documents medication regimen and updates medication profile.
• Collaborates with RN Case Manager, physician, and other team members.
• Provides patient and caregiver education and support.
• Aims to prevent avoidable hospitalizations and ER visits.
