Job summary

National Church Residences in Johnstown, OH seeks an Licensed Practical Nurse to provide nursing care and case management under the plan of care. You will deliver direct patient care, educate families, document per policy, and support home health coordination with the RN team.

The role requires an LPN license in good standing, 12 months of facility/agency nursing experience preferred, and a valid driver’s license for daily travel as part of the home health responsibilities.

Responsibilities
