Job summary

Greater Good Health in Ammon, ID is seeking a Licensed Practical Nurse to join our clinic team. You will collaborate with Nurse Practitioners to provide holistic, high-touch care for a panel of senior patients.

Clinic hours are 7:30-4pm, Monday to Friday, with a 30 minute lunch break. This on-site position focuses on patient experience, care coordination, and adherence to clinical safety standards to deliver value-based primary care for older adults.

Responsibilities

• Deliver outstanding clinical care and support functions related to patient care activities
• Actively engage and manage patient panels in collaboration with the Nurse Practitioner
• Ensure efficient clinic flow by rooming patients promptly and assisting the provider in maintaining a timely schedule
• Conduct visit activities such as taking vitals, performing screenings, recording histories, and preparing patients for examination
• Support post-visit activities, including facilitating referral completion, patient care coordination, and conducting follow-up outreach to patients as needed
• Promote clinical and safety standards to drive high-quality care for our patients
• Maintain a safe, secure, and healthy work environment by establishing and following standards and procedures
