Job summary

Lehigh Valley Health Network is seeking a Licensed Practical Nurse to join a care team focused on improving quality standards, throughput, and patient experience. You will provide clinical and administrative support across the practice and coordinate care with providers and staff.

The role includes collecting patient histories, performing triage, assisting with procedures, and handling patient inquiries about appointments and medications. Day shifts with on-site work are typical.

Qualifications

  • Technical School Diploma completed.
  • LPN program completed.
  • Ability to establish and maintain control of violent patients or dangerous situations.
  • Basic computer skills and strong critical thinking.
  • Knowledge of current nursing practice and patient care standards.

Summary

Works as part of a care team to improve the execution of quality requirements, increase throughput, improve efficiency, and enhance the overall patient experience. Provides care to designated patients and care coordination across the continuum. Performs administrative functions in the practice.


Job Duties


  • Assists provider by collecting patient history and reason for visit.

  • Completes observation, physical exam, telephone triage, and analysis of diagnostic data.

  • Ensures that all ordered laboratory and diagnostic tests are tracked and logged and that the provider is presented with all test results.

  • Provides education to patients, ensures education materials are current and appropriate, and evaluates the effectiveness of patient education.

  • Assists patients with inquiries for prescription refills, appointment scheduling, and referral authorization.

  • Assists providers during procedures and examinations and performs instrument and room clean-up.

  • Coordinates internal and external appointments for new and established patients based on patients' medical needs, preferences, and the practice schedule.

  • Intervenes to prevent, minimize, or correct any actual or potential patient risks based upon observation of physiological, behavioral, and environmental factors.

  • Participates in telephone triage by obtaining clinically appropriate data and symptoms; advises and instructs patients according to established protocols and/or orders from the provider.


Minimum Qualifications


  • Technical School Diploma

  • LPN Program

  • Ability to establish and maintain control of violent patients or dangerous situations.

  • Basic computer skills, critical thinking, and ability to concentrate.

  • Knowledge and competence that reflects current nursing practice.

  • American Heart Association Basic Life Support - State of Pennsylvania

  • Upon Hire LPN - Licensed Practical Nurse_PA - State of Pennsylvania

  • Physical Demands Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
