Job summary

Nephrology Associates of Northern Illinois and Indiana in Fort Wayne, IN is seeking a Licensed Practical Nurse (Phone Nurse) to support patient care by phone, administer medications, and provide wound care and education. You will collaborate with a multidisciplinary team and use the Acumen Epic EHR to document charts.

The role requires a valid Indiana LPN license, a minimum of 3 years in an acute setting (nephrology preferred), CPR certification, and strong communication skills.

Qualifications

• Valid Nursing License in the state of practice.

• Minimum 3+ years clinical experience in an acute setting or equivalent experience.
• Nephrology and/or ambulatory experience preferred.
• Valid CPR certification required.
• Basic computer skills.

• Demonstrated ability to take initiative and work independently, yet also function in a highly collaborative, fast paced, team environment.
• Excellent verbal and written communications skills.

Responsibilities

• Maintains patient charts accurately utilizing the Acumen Epic EHR system for patient documentation.
• Prepares for clinics with any physicians within the practice.
• Orders labs, diagnostic testing, and completes referrals to other physicians, as ordered by NANI physicians.
• Assists office staff with triaging patient clinical phone calls, verifying patient insurance during check-in, and scheduling appointments.
• Manages critical lab values and follows-up with patients and providers as appropriate
• Other duties as assigned.
• Will give health care guidance, assessments, and support to patients primarily over the telephone.
