Community Care, Inc. is seeking a dependable LPN or RN to provide one-on-one in-home care for a 13-year-old patient in the Greensburg area. The patient and family are wonderful to work with.

The nurse will be responsible for monitoring the patient during night shifts while on BiPap therapy.

Shifts Available:

11 p.m. - 7 a.m., 7 nights a week

Position Summary:

The LPN works under the guidance of the RN to provide direct nursing care within agency policies. Responsibilities are based on education, experience, and competency. The LPN does not perform admissions, recertifications, or discharge assessments.

Responsibilities include:
  • Implementing and evaluating skilled and technical patient care activities as directed by the RN.
  • Following the nursing care plan, making suggestions based on patient conditions, and ensuring safety.
  • Administering prescribed medications after proper training and competence verification.
  • Performing treatments ordered by the physician under RN supervision.
  • Observing and reporting on the patient's condition, home environment, and treatment response.
  • Accepting verbal physician orders in accordance with policies.
Documentation:
  • Completing electronic shift and visit notes on the day of service, documenting treatments and responses.
  • Submitting clinical notes promptly via electronic medical records.
  • Completing Time Slip recordings as directed by the RN.
Communication:
  • Reporting patient condition changes and responses to the RN and Case Manager.
  • Maintaining confidentiality per HIPAA regulations.
  • Coordinating care activities with the RN Field Supervisor.
  • Managing supplies and providing availability updates.
Professional Standards:
  • Maintaining professionalism and a positive image.
  • Following infection control and safety policies.
  • Adhering to agency policies and Nurse Practice Act standards.
  • Managing time effectively.
Professional Development:
  • Attending mandatory in-service sessions.
  • Participating in staff meetings, care conferences, and educational activities.
  • Seeking continuous improvement through education and professional involvement.
Additional Duties:

Performs other duties as assigned.

Qualifications:
  • Graduate of a state-approved school with pharmacology coursework.
  • Current Pennsylvania license.
  • Minimum six months of relevant patient care experience.
  • Current CPR certification.
  • Reliable transportation and valid driver’s license with insurance.
  • Clear criminal history, Childline, FBI, DIG, Medicheck, EPLS, and other required clearances.
