Community Care, Inc. is currently seeking a dependable LPN or RN to provide one-on-one care to 4-month-old twins in the Belle Vernon area.

The nurse will be responsible for oxygen monitoring, medication administration, and ADLs during overnight hours. Start and end times are flexible.

Shifts Available:
  • 8 hours/night x 7 nights/week.
  • Overtime available but not required.
Position Summary:

The LPN works under the guidance of the RN as part of a team to provide direct nursing care within agency policies. Responsibilities are based on education, experience, and competency. The LPN does not perform admission, recertification, resumption of care, or transfer/discharge assessments.

Responsibilities include:
  1. Implementing and evaluating skilled and technical patient care activities as designated by the RN.
  2. Following the nursing care plan, making suggestions based on patient conditions, and maintaining safety.
  3. Administering prescribed medications (must have pharmacology training and competence). See Medication Administration Policy.
  4. Performing treatments ordered by the physician under RN supervision.
  5. Observing and reporting on the patient's condition, home situation, and response to treatment.
  6. Accepting verbal physician orders following policy orientation and regulatory guidelines.
Documentation:
  1. Completing electronic shift and visit notes on the day of service, documenting treatments and responses.
  2. Timely submission of clinical notes via electronic medical record.
  3. Completing Time Slip recordings as directed by the RN.
Communication:
  1. Reporting patient condition changes and responses to the supervising RN and Case Manager.
  2. Maintaining confidentiality per HIPAA.
  3. Coordinating care activities with the RN Field Supervisor.
  4. Inventorying and ordering supplies as needed.
  5. Providing monthly availability updates to CCI.
Professional Standards:
  1. Maintaining professional conduct and a positive image.
  2. Following infection control and safety policies.
  3. Adhering to agency policies and the Nurse Practice Act for safe clinical practices.
  4. Managing time effectively.
Professional Development:
  1. Attending annual mandatory in-service education.
  2. Participating in staff meetings, care conferences, and educational sessions.
  3. Seeking ongoing education and professional growth through various means.
Other Duties:

Performing additional duties as assigned.

Qualifications:
  1. Graduate of a state-approved school with pharmacology coursework.
  2. Current Pennsylvania license.
  3. Minimum six months of relevant patient care experience.
  4. Current CPR certification.
  5. Reliable transportation if traveling between assignments, with a valid driver’s license and insurance.
  6. Clear criminal history, Childline, FBI, DIG, Medicheck, EPLS, and other regulatory clearances.
