Job summary

Farmington Wellness & Rehabilitation is seeking a dedicated LVN/LPN to join our night shift team from 6:00 PM to 6:00 AM. We value compassion, clear communication, and high-quality resident care, offering competitive wages, raises and bonuses for performance.

Qualifications

Responsibilities

• Follows established procedures for charting and incident reporting.
• Follows procedures for narcotic counting and pharmacy orders.
• Documents changes in patient conditions including nutrition, infections, skin issues, weight loss or other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in appropriate programs.
• Document and notify departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
