Title: Licensed Practical Nurse – New Providence Women's Clinic

Schedule: Full Time, 35 hours per week

Position Overview:

The LPN will report to the Assistant Director of Nursing Embedded Services and Nurse Manager for Embedded Nursing, and indirectly to the Director of Nursing Operations. The role involves client assignments, daily tasks, and workshop scheduling at the designated shelter. The LPN provides educational resources and workshops on chronic health conditions and promotes health within transitional and permanent housing programs.

  • Manage medication systems for all clients, including:

- Receiving and reviewing medications

- Assisting with refills

- Ensuring safe storage of medications

- Providing clients with supplies and access to medications

- Monitoring reactions and reporting side effects

  • Conduct nursing screenings for new clients
  • Assist in connecting clients with psychiatric, medical, and substance abuse services
  • Participate in weekly clinical meetings with medical staff, case management, and administrative staff
  • Communicate regularly with on-site psychiatric and medical providers, case managers, and administrative staff
  • Coordinate with off-site psychiatric providers as needed
  • Administer medications and injections as ordered
  • Provide medication teaching and support self-administration
  • Offer ongoing health education and support to clients
  • Maintain documentation in accordance with agency policies
  • Assist with tele-psychiatry operations as scheduled
  • Support Primary Care Clinic, Residential, and Substance Use Program coverage as needed

Education Credentials/Requirements:

New York State licensed LPN

Experience:

Experience working with homeless individuals preferred

Preferred Skills:

Communication: Excellent oral and listening skills

Interpersonal Skills: Ability to interface effectively with clients and staff at all levels

Computer Skills: Proficiency in Microsoft Office, especially Word and Excel
