Job summary

McLaren Health Care is seeking an LPN to perform nursing procedures in the home under the supervision of a registered nurse, delivering care within scope of practice.

The role includes wound care, medication administration, catheterization, glucose monitoring, and ensuring patient comfort in a home setting. Requires Michigan license and CPR. Full-time schedule with a standard daytime work window.

Qualifications

• Current licensure with the Michigan State Board of Nursing.
• Two years current nursing experience.
• Current CPR certification.
• Valid driver's license and proof of auto insurance and driving record in accordance with McLaren policy.

Responsibilities

• Performs nursing procedures under direction of a registered nurse, including wound care, irrigation, injections, catheterization, ROM exercises and glucose monitoring.
• Prepares and administers oral, rectal, subcutaneous, intramuscular and topical medications as qualified, observing patient responses.
• Identifies common patient problems and participates in plan of care development with the RN.
• Demonstrates sensitivity to patient comfort, privacy and progress; ensures a safe home environment.

Position Summary

Performs nursing procedures/treatments in the home within scope of practice. Works under the direction of the registered professional nurse.

Essential Functions and Responsibilities

• Performs a variety of basic nursing procedures under the direction of a registered nurse which may include, but are not limited to, wound care, wound irrigations, medication administration, catheterization, range of motion exercises and glucose monitoring.
• If the LPN is appropriately qualified, prepares and administers oral, rectal, subcutaneous, intramuscular, intradermal and topical medications to patients per policy and procedure. Observes patient response to medication, records unexpected reactions and reports them to the registered nurse.
• Identifies common patient problems, symptoms and reactions through monitoring
• Participates in the development, review and evaluation of the plan of care appropriate to the age of the patient in conjunction with the registered nurse.
• Demonstrates sensitivity to patient comfort and privacy and expresses interest in the patient progress.
• Provides for the physical and emotional comfort of the patient as well as a safe home environment.

Required

• Current licensure with the Michigan State Board of Nursing.

• Valid driver's license, proof of valid auto insurance, and maintain a driving record in accordance with McLaren's corporate driving policy and eligibility guidelines.
