Job summary

Sweetwater Care in Wisconsin is seeking an experienced Licensed Vocational Nurse (LVN/LPN) to provide bedside nursing care within scope of practice, under physician orders and center policies.

The role requires charting accuracy, adherence to procedures for narcotics and patient safety, and timely communication of patient condition changes to the care team. Rounds with physicians may be required to ensure quality patient care.

Qualifications

• Long Term Care experience required.
• Point Click Care charting experience required.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional issues, infections, skin conditions, weight loss, or other changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
