Job summary

Attentive Care is actively seeking LPNs for current staffing needs in the Capital Region with multiple shifts across Glendale Nursing Home and Daughters of Sarah. Positions cover 7:00 AM–3:00 PM, 3:00 PM–11:00 PM, and 11:00 PM–7:00 AM, with weekend availability emphasized.

Responsibilities include direct nursing care, med administration, vitals monitoring, wound care, documentation, CNA supervision, and collaboration with the care team.

Qualifications

Hold a current New York State LPN license.
Recent facility-based nursing experience in a nursing home, rehab center, or assisted living setting.
Active BLS or CPR certification.
Eligibility to work in the United States.
Reliable transportation and residence within 25 miles of the facility where you will be working.
At least 6 months of facility-based nursing experience within the past year is a plus.

Responsibilities

Provide direct nursing care to residents according to care plans and physician orders.
Administer medications and treatments safely while monitoring effectiveness and adverse reactions.
Assess, monitor, and document residents' vital signs and health changes.
Perform wound care, dressing changes, and other nursing procedures within the LPN scope.
Maintain accurate documentation in medical records per regulations.
Supervise CNAs and support high standards of resident care.
Collaborate with RNs, physicians, therapists, and care team.
Respond to medical emergencies and infection control protocols.
Promote resident dignity, comfort, safety, and independence.
Adhere to state/federal regulations and facility policies.

Responsibilities

Provide direct nursing care to residents in accordance with individualized care plans, physician orders, and facility policies.
Administer prescribed medications, treatments, and delegated nursing procedures accurately and safely while monitoring for effectiveness and adverse reactions.
Assess, monitor, and document residents' vital signs, physical condition, and changes in health status, promptly reporting significant findings to the Registered Nurse or attending provider.
Perform wound care, dressing changes, specimen collection, catheter care, and other nursing procedures within the LPN scope of practice.
Maintain accurate, timely, and complete documentation in the electronic or paper medical record in accordance with regulatory and facility requirements.
Supervise and support Certified Nursing Assistants (CNAs) by providing direction, monitoring delegated tasks, and promoting high standards of resident care.
Collaborate with Registered Nurses, physicians, therapists, and the interdisciplinary care team to ensure coordinated, high-quality resident care.
Respond appropriately to changes in resident condition, medical emergencies, and infection prevention protocols while following established clinical procedures.
Promote resident dignity, comfort, safety, and independence while building positive relationships with residents, families, and fellow healthcare professionals.
Adhere to all applicable state and federal regulations, facility policies, infection control standards, and patient confidentiality requirements

Requirements

Hold a current New York State LPN license
Recent facility-based nursing experience in a nursing home, rehab center, or assisted living setting, including recent facility staff relief experience
Active BLS or CPR certification
Eligibility to work in the United States
Reliable transportation and residence within 25 miles of the facility where you will be working
At least 6 months of facility-based nursing experience within the past year is a plus
