Job summary

Hartford HealthCare is seeking a licensed Practical Nurse (LPN) to provide direct patient care under RN or Provider supervision across units, with travel to other urgent care locations as needed. Qualifications

• Current Connecticut LPN license is required.

Responsibilities

• Direct patient care to meet individual health goals and document data.
• Obtain, document, and report patient data related to the plan of care.
• Perform therapeutic measures under the direction of an RN or Provider.

Shift Detail: Monday–Friday: Shift availability between 8:00 AM–10:00 PM, with flexibility in assigned shift times. One weekend: 8-hour shift per month.

Job Summary

Under the supervision of a Registered Nurse, or Provider, the LPN provides quality and cost-effective direct care for patients/families within a unit, service or other primary area of practice.

Travel flexibility: Must be willing and able to travel to other urgent care locations, as needed, to provide coverage.

• Collaborates with members of health care team to assist patients in achieving established outcomes of care
• Participates in direct patient care in order to meet individual and family health goals by: obtaining, documenting and reporting patient data related to established plan of care
• Performs therapeutic measures, under the direction of an RN or Provider, to assist in achieving established patient care outcomes.
