Senior Lifestyle Corporation is seeking a licensed practical nurse (LPN) for a full-time evening shift in our skilled nursing community. The role focuses on medication administration, resident assessments, and care planning, with a strong emphasis on teamwork and safety.

The ideal candidate holds a current LPN license in Illinois, is CPR certified, and has 1+ year of experience in senior care. A friendly, professional approach to resident care and strong communication skills are essential.

Qualifications

• Must hold a current Licensed Practical Nurse license in the practicing state.
• CPR certification required.
• Must be able to communicate effectively in English with residents, families and staff.
• Basic computer skills for documentation and communication.

Responsibilities

• Administer medications to residents in accordance with Federal & State regulations and company policies.
• Assist with admissions including assessments and documentation as needed.
• Develop and implement acute and long-term care plans with residents and families.
• Monitor health status and report changes to physicians and supervisors.
• Provide health-related consultations and respond promptly to emergencies.
• Maintain confidentiality of resident information and comply with Safety & Disaster Plans.
• Educate residents and staff on health topics as required.

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Understand roll in the Safety & Disaster Plan.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May be responsible for discharges, assessments, and clinical documentation as needed.

Level of Formal Education: Training from an accredited licenses or vocational nursing program

Years of Experience: 1+ years
Type of Experience: Senior care or related field
Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified
