Job Summary

The primary function of the hospice Licensed Practical Nurse (LPN) in the Shenandoah House is to provide nursing care under the supervision of a registered nurse for patients of all ages and coordinate care with the hospice IDT, patient, and family. The candidate must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served at the Shenandoah House and be able to provide the care needed as described in the department's policies and procedures. This position plays a critical role in supporting Augusta Health’s mission and advancing departmental goals through measurable performance indicators and service excellence. Essential Job Duties
  • Practice within the VA Nurse Practice Act and LPN scope of practice.
  • Ensure compliance with HIPAA regulations to maintain patient confidentiality.
  • Demonstrate respect for patient dignity, with no reported breaches or complaints from patients or fellow care providers.
  • Perform data collection and nursing interventions safely, following physician orders.
  • Continuously reassess patients and promptly report changes in condition to the RN.
  • Provide patient teaching and follow-up care as appropriate.
  • Support patients in Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
  • Involve family members in care and patient education.
  • Maintain a clean, safe, and supportive environment.
  • Prepare light meals as needed.
  • Coordinate patient care with other healthcare providers and interdisciplinary team (IDT) members, ensuring accurate handoff communication.
  • Consistently follow CDC hand hygiene guidelines and utilize personal protective equipment (PPE) appropriately.
  • Demonstrate knowledge of safe patient handling techniques and equipment.
  • Identify fall risks and implement preventive measures.
  • Use patient two-identifier verification consistently.
  • Appropriately label specimens at the bedside.
  • Ensure 100% compliance with the Five Rights of Medication Administration.
  • Demonstrate knowledge of all prescribed medications, including high-risk drugs.
  • Maintain secure medication storage and handling.
  • Assess pain relief after medication administration and promptly report unrelieved pain to the RN.
  • Participate in quality improvement projects and staff satisfaction initiatives.
  • Contribute to the Quality Assurance and Performance Improvement (QAPI) plan.
  • Provide input on cost containment and resource management.
  • Maintain readiness for surveys and ensure compliance with environmental and policy standards.
  • Use safety devices, shields, and proper body mechanics to minimize injury risks.
  • Dispose of sharps safely and correctly.
  • Complete all annual education and department competency requirements.
  • Takes responsibility for ongoing professional development and learning.
  • Maintain a positive attitude and adaptability within the team.
  • Demonstrate reliable attendance and minimize scheduling disruptions.
  • Attend staff meetings and in-service training sessions as required.
  • Support the hospice philosophy and delivery of compassionate end-of-life care.
  • Act as a mentor to new staff and students when needed.
  • Demonstrate collaboration and cooperation with the interdisciplinary team to meet patient needs.
Required Qualifications
  • Education: Graduate of an Accredited School of Vocational/Practical Nursing.
  • Licensure/Certification:
    • Current Commonwealth of Virginia license as a Licensed Vocational/Practical Nurse, or a multi-state license.
    • CPR- Basic Life Support Healthcare Provider certification (BLS HCP).
  • Experience: Medical/Surgical experience in hospital or clinical setting within the last year.
  • Driver’s License: Yes
    • To drive an Augusta Health Vehicle, must also be 21 years old and Motor Vehicle (MVR) must be approved by Risk Management.
  • Eligibility to work in the United States and meet Virginia state employment requirements.
