Licensed Practical Nurse (LPN), Senior Services, Part-Time (Day Shift)

Join us at Penn Medicine, University of Pennsylvania Health System as a Licensed Practical Nurse (LPN), Senior Services, Part-Time (Day Shift).

**Location:** Lancaster, PA

**Hours:** Part-Time, Day Shift (0.5 FTE/20 hours a week). Schedule includes Monday & Thursday from 8:45am-5:15pm; Wednesdays 1:00pm-5:00pm.

**Sign-on Bonus:** $2,500, based on FTE status.

Job Overview

Provide nursing services to geriatric patients in a provider’s office. Responsibilities include participating in Geriatric Consults and Annual Wellness Visits, patient education, administering medications, assisting with procedures, performing Point of Care Testing, and coordinating care activities.

Essential Functions
  • Offer clinical advice via phone/electronic correspondence, following protocols and consulting with providers as needed.
  • Educate patients on preventive care, chronic disease management, medications, and more.
  • Prepare patients for visits, assist with procedures, and perform tests and specimen collections.
  • Manage prescriptions, track test results, and perform wound care as necessary.
  • Maintain cleanliness and supply of exam rooms and medical instruments.
  • Review and update care plans, follow up with patients, and assist with proactive outreach.
  • Coordinate referrals, education, and community resources.
  • Participate in performance improvement initiatives and daily provider huddles.
  • Act as a patient experience advocate, ensuring professional communication and confidentiality.
Secondary Responsibilities
  • Perform front office duties as needed.
  • Travel regionally if required, with a valid driver’s license.
  • Participate in on-call duties as assigned.
Qualifications
  • Completion of an LPN or RN educational program.
  • Current PA licensure as RN or LPN.
  • AHA CPR/BLS certification within 30 days of hire.
Preferred Experience
  • 1-2 years in primary or specialty care settings.
  • Experience in triage, emergency, cardiac, or ICU nursing.

  • Ability to focus on multiple tasks, learn and retain information, and solve problems based on established guidelines.
  • Perform under stress, adapt to changing duties, communicate effectively, and maintain interpersonal relationships.
Physical & Environmental Requirements

Requires frequent walking, standing, and reaching; occasional bending, twisting, and lifting up to 50 pounds; exposure to biohazards and bloodborne pathogens.

Must have vision and hearing abilities necessary for patient care.
