Job summary

Lifepoint Health, Inc. is seeking a Rehabilitation Licensed Practical Nurse (LPN) for a full-time daytime role in Winchester, TN.

You will work on a 12-hour shift schedule (3x12) with rotating weekends and holidays, providing direct patient care on our 12-bed inpatient rehab unit and collaborating with a multidisciplinary team to deliver patient-centered care. Applicants should have an active Tennessee LPN license and prior experience in rehabilitation or acute care settings.

Qualifications

• Licensed Practical Nurse with TN license (compact or single-state)
• Graduate of a Practical Nursing program
• AHA BLS certified within 30 days of hire
• One year of prior nursing experience preferred

Responsibilities

• Provide direct patient care under RN supervision per scope of practice
• Assist in assessment and data collection for plan of care
• Perform plan of care interventions including wound care and specimen collection
• Administer medications under supervision as allowed by state law
• Assist patients with activities of daily living and mobility
• Monitor patient status and communicate updates to RN/physicians/families
• Advocate for patient physical and emotional well-being

We are a 12-bed inpatient rehabilitation unit and the only acute inpatient rehab option in the area, serving a diverse population with a wide range of diagnoses. Our team practices a strong interdisciplinary approach, allowing nurses to work closely with therapy, physicians, and case management to deliver comprehensive, patient-centered care. We maintain a supportive work environment with an average nurse-to-patient ratio of 1:2-5, promoting quality care and professional growth.

How you'll contribute

A Rehabilitation LPN who excels in this role:

• As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.
• Under the direction of an RN or Rehab Charge Nurse, contributes to the assessment of patients by collecting data for analysis by an RN.
• Performs plan of care interventions, including specimen collection and wound care. May administer medication. Documents patient care given.
• Assists RN with development of plan of care.
• Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
• Works as an advocate for the physical and emotional well-being of the patient.

What we're looking for

Applicants should have an Active Tennessee license (single-state or compact) and be a graduate of a Practical Nursing program. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins. Additional requirements include:

• Certifications: AHA Basic Life Support (BLS), within 30 days of hire
• Required Skills: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Experience: One-year previous experience preferred.
• Licenses: LPN License for the State of Tennessee.
• Education: Graduate of a Practical Nursing program.
