Job summary

The Lifepoint Health-affiliated HSHS Rehabilitation Hospital is seeking a Full-Time Nurse (LPN/LVN) to deliver direct and indirect patient care using the nursing process. You will assess, plan, intervene and evaluate care, develop care plans with patients and families, and communicate changes to the medical team.

You’ll participate in safe discharge planning and coordinate with interdisciplinary staff to ensure smooth transitions.

Qualifications

  • Graduation from an accredited nursing program (BSN/ADN or Diploma).
  • Current state licensure as Licensed Practical Nurse.
  • CPR/BCLS certification, ACLS preferred.
  • Minimum six months’ Med/Surg experience in an acute care setting.
  • Excellent oral and written communication and interpersonal skills.

How You'll Contribute

Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient's clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharges planning process.

  • Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and Evaluation
  • Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans
  • Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues
  • Play a key role in the discharge planning process to support safe, successful transitions

What We're Looking For

Applicants should possess the following qualifications and skills:

  • Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
  • Current state licensure as Licensed Practical Nurse.
  • CPR/BCLS certification. ACLS preferred.
  • Minimum six months' Medical/Surgical experience in an acute care setting preferred.
  • Excellent oral and written communication and interpersonal skills.
