Job summary

Community West Rehabilitation Hospital in Brownsburg, IN is seeking a Licensed Practical Nurse (LPN) for night shifts on a full-time basis. The position covers 36 hours per week with 12-hour shifts from 7:00 PM to 7:30 AM.

The role includes direct patient care using the nursing process under RN supervision, with emphasis on rehab nursing in a multidisciplinary setting. CPR/BCLS required; ACLS preferred; Indiana LPN license is mandatory.

Qualifications

• Requires Indiana LPN license and current CPR/BCLS; ACLS preferred.
• Strong interpersonal and documentation skills.
• Familiarity with rehab documentation systems is preferred.

Responsibilities

• Demonstrates knowledge of medications and correct administration per patient age and condition.
• Performs care considering age-specific needs for geriatric and rehab inpatients.
• Under RN supervision, assists with patient assessments and reassessments; initiates rehab interventions.
• Under RN supervision, performs timely and accurate quality improvement assessments.
• Follows plan of care and updates interdisciplinary care plan as patient condition changes.
• Interacts professionally with patient and family to form the care plan.
• Educates patient and family on pain management and participates in pain control.
• Other duties as assigned

How you'll contribute

A Licensed Practical provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.

Essential Functions

• Demonstrates knowledge of medications and their correct administration based on the age and clinical condition of the patient
• Performs patient care considering needs specific to the standard of care for patient's age; includes geriatric and general inpatient rehabilitation patients
• Under the supervision of the RN, assists with the assessment on all patients and reassessments as per policy.Identifies and initiates appropriate rehabilitation nursing interventions
• Under the supervision of the RN, performs timely and accurate QI assessments
• Carry out the plan of care as indicated by the patient's needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly based on changes in patient's condition and individual needs
• Interacts professionally with patient/family and involves patient/family in the formation of the plan of care
• Demonstrates ability to adequately assess and reassess pain.Utilizes appropriate pain management techniques.Educates the patient and family regarding pain management
• Other duties as assigned

What we're looking for

Applicants should have a Licensed Practical Nurse license in the state of Indiana. Additional requirements include:

• Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.
• CPR/BCLS certification. ACLS preferred.
• Excellent oral and written communication and interpersonal skills.
