We are looking for qualified LPNs to provide high-quality care for our home health patients in a professional and compassionate manner. You will use assessment tools to manage patients' medical needs and identify other disciplines needed to maximize outcomes. As a patient advocate, you will communicate with the primary doctor, Clinical Team Manager, and assigned Registered Nurse, depending on the patient’s condition.
