We are hiring an LPN PRN for Home Visits!

The Licensed Practical Nurse in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.

Responsibilities

• Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team. May not alter the plan of care.
• Instructs patients' families in patient care and maintaining a healthy environment.
• Observes, records, and reports to the supervising nurse and/or physician the patient's responses to treatment and any changes in condition daily.

Education and Experience

License Requirements

• Current LPN licensure in the state of practice.
• Valid Driver's License, vehicle insurance, and access to a dependable vehicle or reliable public transportation.
• CPR Certification required.
