Job summary

Signature HealthCARE of Fentress County is seeking a dedicated LPN/LVN to provide direct nursing care to residents in a 140-bed facility. You will assist with ADLs, medication administration, wound care, and participate in care planning and documentation with an interdisciplinary team.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• "CPR/BLS certification preferred"
• "Minimum one (1) year of clinical nursing experience; long-term care experience preferred"
• "Experience in a supervisory or charge nurse role preferred"
• "Knowledge of Federal and State long-term care regulations"
• "Strong verbal and written communication skills"
• "Ability to work independently, prioritize tasks, and exercise sound clinical judgment"
• "Familiarity with electronic health records (EHR) systems"
• "Proficiency with Microsoft Office applications"

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents' physical and emotional status; notify the attending physician and responsible party of any change.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

How You Will Make a Difference

What You Need To Make a Difference

• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
