Job description

What you will do in this role:
  1. Demonstrate knowledge of medications and their correct administration based on the patient's age and clinical condition.
  2. Perform patient care considering the needs specific to the standard of care for the patient’s age, including geriatric and general inpatient rehabilitation patients.
  3. Assist with patient assessments under the supervision of the R.N., conducting reassessments as per policy, and identify and initiate appropriate rehabilitation nursing interventions.
  4. Perform timely and accurate quality improvement (QI) assessments under the supervision of the R.N.
  5. Carry out the plan of care based on patient needs and response to treatment; evaluate the overall plan daily for effectiveness and update the interdisciplinary care plan accordingly.
  6. Interact professionally with patients and families, involving them in the formation of the plan of care.
  7. Assess and reassess pain adequately, utilize appropriate pain management techniques, and educate patients and families regarding pain management.
  8. Perform other duties as assigned.
Minimum Qualifications:
  1. Graduate of an accredited licensed practical/vocational nurse program.
  2. Current state licensure as a Licensed Practical/Vocational Nurse.
  3. CPR/BCLS certification; ACLS preferred.
  4. IV Certification required or obtained within 6 months; highly preferred.
  5. Minimum of six months’ medical/surgical experience in an acute care setting preferred.
  6. Excellent oral and written communication and interpersonal skills.
