The role offers flexible scheduling (FT, PT, PRN) with days, nights, and weekend shifts, plus 24/7 support and ongoing education opportunities.

Qualifications

• Active, unrestricted LPN license (PA).
• License acquired 1 year prior to application.

Responsibilities

• Provide skilled nursing care in a home health setting.
• Perform ongoing patient assessments and clinical evaluations.
• Administer prescribed medications, treatments, and therapies.
• Deliver care in accordance with physician orders and plan of care.
• Coordinate care with families, providers, and care teams.
• Educate patients and family members to support improved outcomes.
• Implement preventive measures to ensure patient safety and quality care.

What You’ll Do

Schedule & Flexibility
