3- 12/hr shifts- 7p-7a

Job Summary:

Provide supportive care for residents under the direction of the Enhanced Living Director of Nursing and clinical manager; provides clinical leadership to the Neighborhood Care Teams.

Job Duties/Responsibilities:

  • Help orient new residents to the Enhanced Living neighborhood

  • Provide supervision and care to residents to maximize their capabilities and independence

  • Assist Enhanced Living residents with medications per physician's orders; following DHS guidelines and policies/procedures related to medication

  • Document all medications administered to residents on their EHR

  • Make reasonable measures to ensure that residents take their medications

  • Document refusal of medications on the EHR

  • Notify resident's Primary Care Physician of medication refusal as indicated by physician

  • Assist the Enhanced Living Health Information Assistant (HIA) to ensure that residents are notified when yearly physical examinations are due

  • Assist the HIA in coordinating transportation for residents as needed

  • Observe residents in daily activities and keeps Enhanced Living Director of Nursing informed of unusual problems

  • Actively participates in orientation and training of new team members

  • Provide clinical leadership to Neighborhood Care Teams as needed and keeps team members informed of issues that require attention

  • Reorder medications for residents as necessary

  • Coordinate tray services for residents who are unable to have meals in the dining room

  • Familiar with the resident handbook and Village policies and procedures

  • Maintain good public relations for the organization

  • Attend RASP Meetings when appropriate and/or provide feedback for RASP updates

  • Maintain resident charts

  • Work to enhance quality of life and independence for all Enhanced Living residents

  • Follow emergency procedures and respond rapidly and appropriately to emergencies ensuring the safety of the residents and team members

  • Follow proper procedures and promote safe work practices in order to maintain a safe working environment

  • Work cooperatively and productively with others, including residents, families, and other team members

  • Willingness to work under direction and supervision

  • Confidentiality regarding resident information must be maintained in strict confidence, as well as other information pertaining to the business of Messiah Lifeways

  • Willingness to assist with covering parts of shifts and/or different shifts as necessary during staffing concerns as part of the team

  • Familiar with the guidelines established by DHS Personal Care Home Regulations, especially as it relates to Medical/Medication issues and works within those guidelines

  • Additional duties and tasks as assigned by the Enhanced Living Director of Nursing

  • Administers insulin, and performs blood glucose monitoring according to P and P of Messiah

  • Ability to perform a straight catheter procedure, and foley catheter insertion/removal under sterile technique . The ability to flush a foley catheter for blockage.

Qualifications

Required Skills/Abilities:

  • Discretion, tact, patience, and compassion are required to perform effectively

  • Competent with Google Suite Applications and Electronic Health Records

Education/Experience:

  • LPN/RN Graduate from an approved school

  • Three (3) years nursing experience with a minimum of one (1) year in gerontology preferred

  • CPR certified unless contraindicated for health reasons
