Job summary

Cayuga Health System is seeking a Licensed Practical Nurse (LPN) for per diem, flexible shifts at Seneca View Skilled Nursing Facility in Montour Falls, NY. You will support assessment, planning, implementation, and evaluation of patient care across all ages during shifts.

Responsibilities include administering medications, recording vitals, coordinating with the care team, and documenting in the EMR system. NYS LPN licensure and CPR/BLS are required; weekend rotation is expected.

Qualifications

• Graduate of a Nursing Program.
• Current NYS LPN License.
• CPR/BLS certification.

Responsibilities

• Administers medications and treatments under supervision of an RN in accordance with Facility Policy.
• Taking the patient's vital signs (respiration, temperature, blood pressure, heart rate) and reporting changes.
• Practices ethically and professionally within scope; advocates for patient.
• Maintains clinical competence and completes required education within set timeframe.
• Coordinates care with interdisciplinary team.
• Documents appropriately and thoroughly in EMR/Point Click Care.
• Keeps RN and other nursing staff informed about changes in condition.
• Participates in unit councils and staff meetings.
• Weekend rotation required.

The primary functions of an LPN in assisting with assessment, planning, implementing and evaluating the care of all patients, regardless of age, during his/her shift. Responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. Effectively provides Person Centered care and services.

Roles and Responsibilities

• Administers medications and treatments under the supervision of a Registered Professional Nurse in accordance with Facility Policy.
• Taking the patient's vital signs such as respiration, temperature, blood pressure, and heart rate and reporting changes in the patient's /residents conditions
• Practices ethically and professionally within their scope; advocates for patient
• Maintains clinical competence specific to their care area including organization required education within stated timeframe
• Coordinates care with interdisciplinary team
• Documents appropriately and thoroughly in EMR/Point Click Care
• Keeps other nursing personnel and responsible RN informed regarding changes in condition of patients/ residents and / or other pertinent observations
• Participates in unit councils and staff meetings.
• Weekend rotation required

Required Skills and Experience

• Graduate of a Nursing Program
• Current NYS LPN License
• CPR/BLS

Preferred Skills and Experience

• Prior patient care and customer service experience

Physical Requirements

• May require lifting (up to 50 lbs.), may require sitting or standing for long periods. Must be able to physically operate the equipment required for the job.

Pay Range Disclosure

• $26.15 to $37.00 per hour
