Matrix Home Health Care is seeking experienced Licensed Practical Nurses (LPN) to perform home health visits in the Downtown Miami area. The role emphasizes initiating care, completing comprehensive assessments and ensuring accurate OASIS documentation for home health patients.

You will provide home-based nursing to patients recovering from hospitals, injuries or chronic conditions while coordinating with physicians and care teams.

Qualifications

• Active Florida LPN license in good standing.
• Minimum 1 year of home health experience.
• Strong OASIS documentation experience.
• KanTime EMR experience preferred.
• Excellent clinical assessment and critical thinking skills.
• Ability to work independently with a flexible schedule.
• Reliable transportation and valid driver’s license.

Responsabilités

Job Description

Matrix Home Care is seeking experienced and compassionate Licensed Practical Nurses (LPN) to perform home health visits in the Downtown Miami area . This role is critical in initiating patient care, completing comprehensive assessments, and ensuring accurate OASIS documentation for home health patients.

• Provide home-based care to patients with illnesses, injuries, and chronic conditions, often following discharge from a hospital, rehabilitation center, or nursing facility


• Examine patients, complete clinical assessments, document progress, and report changes to physicians


• Complete accurate and timely clinical documentation, including OASIS, evaluations, physician orders, and care coordination notes


• Develop, implement, and continuously monitor patient care plans in collaboration with patients and their representatives


• Deliver skilled nursing services and provide case management for assigned patients


• Educate patients on plan of care, medications, therapies, diet, and exercise


• Coach and support family members on patient care, medication management, and home safety


• Coordinate care with physicians, discharge planners, and interdisciplinary teams


• Oversee and collaborate with LPNs, CNAs, and home health aides providing patient care


• Monitor patient progress, outcomes, and changing needs, adjusting care plans accordingly


• Ensure care goals are met and coordinate appropriate discharge from services


• Serve as a liaison with community health resources, including DME providers, oxygen services, and infusion therapy providers

Requirements

Qualifications

• Active Florida Licensed Practical Nurse (LPN) in good standing


• Minimum 1 year of home health experience required


• Strong experience withOASIS documentation


• Experience with KanTime EMR system preferred


• Excellent clinical assessment and critical thinking skills


• Ability to work independently and manage a flexible schedule


• Reliable transportation and valid driver’s license
