Job summary

CATCH Inc. is seeking a Licensed Practical Nurse (LPN) for Penrose Place in Philadelphia. The role focuses on medication management, resident health support, and care coordination within a 16-bed long-term structured residence.

Part-time weekend openings are available and involve administering medications and coordinating care. Applicants must hold a Pennsylvania LPN license and have experience with mental health or residential settings.

Qualifications

  • Current, active, and unrestricted Pennsylvania Practical Nurse license.
  • Experience supporting adults with serious mental illness; residential behavioral health experience strongly preferred.
  • Working knowledge of medication administration, health assessment, clinical documentation, crisis response, infection control, and resident education within the LPN scope.
  • Clear communication, sound clinical judgment, strong organization, and the ability to recognize and upscale changes in condition.

Responsibilities

  • Assess and monitor residents' physical and behavioral health status, including vital signs, symptoms, medication response, and changes in condition.
  • Administer medications and treatments as ordered and maintain accurate medication administration records and nursing progress notes.
  • Communicate medical or psychiatric concerns to the appropriate nurse, physician, prescriber, or program leader and follow through on instructions.
  • Coordinate outside medical appointments, testing, follow-up, transportation, and communication with providers and the treatment team.
  • Provide resident and staff education regarding medications, side effects, hygiene, nutrition, first aid, health maintenance, and self-management.
  • Respond to medical and psychiatric emergencies within scope and participate in treatment-team meetings, quality reviews, and required training.

Licensed Practical Nurse (LPN) - Penrose Place

Medication management, resident health support, and care coordination

$35.00 per hour

Part-time weekend openings are available

The Licensed Practical Nurse provides psychiatric and medical nursing support to residents of Penrose Place, a 16-bed Long-Term Structured Residence. Working within the LPN scope of practice, you will administer medications and treatments, monitor changes in condition, coordinate care, and help residents understand and manage their health needs.
