Job Summary

We are seeking a Licensed Practical Nurse to join our team. In this role, you will be caring for Hospice patients in their homes. You are a Licensed Practical Nurse (LPN) with great attention to detail and a big heart. You take pride in caring for patients following personalized care plans. You are able to assess, observe, and communicate effectively regarding your patients' care.

Responsibilities

• Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans.
• Administer medications and monitor patients for side effects or reactions.
• Document any changes in patient conditions and report on changes.
• Maintain accurate records of patient care and discuss observations with the supervisor.

Qualifications

• Currently licensed Licensed Practical Nurse (LPN) in the state of Ohio.
• Previous experience as a Licensed Practical Nurse is preferred.
• Hospice experience preferred but not required.
• Commitment to excellent care and customer service.
• Excellent interpersonal and client care skills.
• Valid driver’s license.
• First aid/CPR certification required.
