Job summary

ReMed, a Collage Rehabilitation Partner, is seeking a Part-Time LPN to join our residential program in Malvern and West Chester. The LPN provides nursing and facilitation services for clients within designated programs, under supervision of a Registered Nurse.

You will implement evidence-based strategies to promote physical, psychosocial, and spiritual health, and train families or caregivers on health maintenance and medical stability.

Qualifications

• State licensure as a Licensed Practical Nurse (LPN) is required.
• Experience in rehabilitation of chronically or catastrophically ill or injured is recommended.
• Demonstrated accountability and problem solving skills.

Responsibilities

• Utilize nursing knowledge and skills to provide basic preventive, therapeutic and rehabilitative care.
• Completes timely identification of individuals needing medical nursing services or interventions.
• Provides nursing interventions as needed.
• Trains/educates families, funders, and other constituents on health maintenance and discharge planning.
• May support or provide direct care to clients at program locations or during medical appointments.

Job Summary

We are searching for a Part-Time LPN to work in our residential program located in Malvern and/or West Chester. The Licensed Practical Nurse (LPN) typically works under the direction of a Registered Nurse and provides nursing and facilitation services for assigned clients within each designated program(s). The LPN works as part of the clinical team to implement evidence-based treatment strategies that are related to self-care and that promote physical, psychosocial, and spiritual health. The LPN also provides training to clients or family members regarding how to promote health, wellness and medical stability.

Essential Duties & Functions

• Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned client as evidenced by documentation and observation of positive client care outcomes
• Completes timely identification of individuals in need of medical nursing services or interventions.
• Provides interventions as needed
• Possesses the specialized knowledge and clinical skills necessary to provide care for clients; performs hands on nursing care and facilitator care by using nursing process
• Trains/educates families, funders, employers, and other constituents, and uses appropriate resources to develop and implement an individualized teaching and discharge plan
• May support or provide direct care to clients at program locations or while on medical appointments as needed

• Experience in the rehabilitation of chronically or catastrophically ill or injured is recommended
• Demonstrated accountability and skills in analysis, problem solving
