Reflections Retirement Community is seeking a part-time LPN for the 2nd shift at Grand Victorian of Crystal Lake, Crystal Lake, IL. The role involves medication administration per physician orders and strict adherence to documentation to ensure resident safety.

You will participate in personalized service planning, promote resident independence and collaborate with physicians, staff, and families while contributing to quality improvement efforts and ongoing education opportunities.

Qualifications

• Current state licensure as Licensed Practical Nurse (LPN).
• Current CPR certification.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

• Oversees medication distributionas stated on physician orders, ensuring strict adherence to Community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety
• Upon thorough assessment, creates personalized service plans tailored to individual care needs while championing resident independence and dignity
• Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of communication pertaining to the physical, mental, and social well-being of our residents
• Contributes toward quality assurance and performance improvement initiatives
• Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experience
