Salary Range: $31.50 - $32.00 Hourly

Job Shift: 2nd shift

Responsibilities

• Oversees medication distribution as stated on physician orders, ensuring strict adherence to Community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety
• Upon thorough assessment, creates personalized service plans tailored to individual care needs while championing resident independence and dignity
• Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of communication pertaining to the physical, mental, and social well‑being of our residents
• Contributes toward quality assurance and performance improvement initiatives
• Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experience

Qualifications

• Current state licensure as Licensed Practical Nurse (LPN)
• Current CPR certification
• Exceptional communication and interpersonal skills
