Job description

We are open to either Licensed Practical Nurse (LPN) OR Medical Assistant (MA/CMA/RMA) is a 1.0 FTE (40 hours/week), at the Phalen, St. Paul Clinic. Scheduled Monday - Friday, 7:30am - 5:30pm with some flexibility needed.

Licensed Practical Nurse (LPN) position exists to administer family-centered nursing care to patients, advise patients on health maintenance and disease prevention, and/or provide skilled assistance in navigation of the health care system as appropriate to the developmental, cultural, emotional, and medical, surgical, or rehabilitative needs of the patient and their families. Nurses’ duties and title are determined by their work setting or patient population served. The role and scope of Licensed Practical Nurses (LPNs) is aligned with the Minnesota Board of Nursing and the Nurse Practice Act (MN Statute Section 148.171). Specific duties are determined by the work setting or patient population served; however, in all positions, serving patients with challenging and complex conditions requires a concentration of specialized staff able to work in highly coordinated teams. We organize care around each patient’s condition which requires staff at multiple levels and across many departments to exhibit exceptional teamwork qualities including communicating efficiently and effectively and adopting the behaviors required for high functioning.

Medical Assistant (MA/CMA/RMA) (only) Sign on Bonus $2,500.Medical Assistant (MA/CMA/RMA) position is expected to maintain and promote excellent patient care and customer service by supporting the efficient running of the clinic through direct patient care clinical duties and administrative duties. Specific duties are determined by the work setting or patient population served as directed by providers (MD/APP) and nurses but in coordination with the entire patient care team. We organize care around each patient’s condition which requires staff at multiple levels and across many departments to exhibit exceptional teamwork qualities including communicating efficiently and effectively and adopting the behaviors required for high functioning integrated teams.

Compensation & Benefits Information

The hourly wage for Licensed Practical Nurse (LPN) is $24.85 to $34.48, with a median wage of $29.66. Pay is dependent on several factors including relevant work experience, education, certification & licensure, and internal equity.

The hourly wage for a Medical Assistant (MA) is $21.00 to $29.53, with a median wage of $24.61, or $21.00 to $31.30 with a median wage of $26.09 if you are certified (CMA/RMA). Pay is dependent on several factors including relevant work experience, education, certification & licensure, and internal equity.

Core Responsibilities and Duties as a Licensed Practical Nurse (LPN):

Patient Care – Delivers patient care which optimizes the patient’s health status and outcomes through the use of the nursing process.

  • Demonstrates thorough focused assessments.
  • Collaborates in the development and modification of individualized plans of care that are evidence-based.
  • Successfully implements interventions within a plan of care or as delegated/assigned by a registered nurse.

  • Communicates patient-related nursing data and information to enhance active and informed decision making by patients, families, nurse colleagues, and other healthcare providers.

Family centered care - Provides nursing care and health education reflecting specific patient, family, cultural and community needs.

  • Implements an effective teaching plan.
  • Employs strategies to promote health in a safe environment.
  • Seeks opportunity to evaluate effectiveness of health education as culturally and developmentally appropriate.

Autonomous Practice – Exercises independent judgment that influences care and demonstrates understanding of potential clinical implications.

  • Assigns nursing activities/tasks to other LPNs or unlicensed personnel, and provides care based on the abilities and limitations of care personnel and patient acuity.
  • Identifies and incorporates evidence based improvements into own daily work.

Procedures and Policies –Identifies, interprets, and implements organizational policies and procedures according to regulations, professional and community standards.

  • Protects the confidentiality of personal health information.

Active Engagement – Actively represents the department in committees, task forces, unit activities or other work groups and disseminates information and decisions to colleagues and peers using the Engaged Professional Practice Model. This includes attending a minimum of 75% of department staff meetings.

  • Ensures accountability through the use of dashboards and promote transparency through reporting of near misses and medical errors.
  • Demonstrates meaningful contributions to the productivity of the professional environment of the department.
  • Supports work of Nursing Collaborative Governance initiatives.

Core Responsibilities and Duties as a Medical Assistant (MA/CMA/RMA):

Provides direct support to the providers during patient visits.

  • Works as an integral part of the clinical team to provide consistent delivery of quality patient care by collecting pertinent medical information (i.e. medical history, allergies, medications, falls risk, depression) in the patients’ electronic medical record documenting, explaining treatment procedures, preparing patients for examination, assisting providers during exams and collecting and preparing laboratory specimens.
  • Accurately evaluates provider orders and is able to effectively communicate with providers when clarifications are needed.
  • Provides patient care in accordance with Gillette, Joint Commission and quality standards.
  • Communicates patient-related data and information to enhance active and informed decision making by patients, families, nurse colleagues, and other healthcare providers.

Demonstrates proficient access to Gillette clinical information systems and completes documentation in an accurate, thorough, and timely manner.

  • Completes documentation in an accurate, neat and timely manner, effectively detailing the events of the appointment including rooming patients, initial screening (falls, behavior, PROM surveys, etc.), gathering chief complaint, clinical measurement data (Height/weight, vital signs, etc.), obtain electrocardiogram, procedure support (passing instruments, supporting patient, positioning, etc.), pre-procedure instruction phone calls, reinforce, administer immunizations patient/family instructions and details for follow up.

Maintains efficient patient flow throughout the clinic experience.

  • Ensures individual work areas and exam rooms are regularly cleaned, stocked and kept orderly to promote a safe and efficient work area.
  • Maintains supplies and equipment throughout the clinic environment.

Supports the clinical team in patient care coordination and communications.

    • Prepares pre-visit documentation for clinic visits.
    • Answering Phones
    • Assists with communication and correspondence.
    • Manages patient information received from outside resources.
    • Documents using EMR and other organizational tools.
    • Tracks lab/imaging results and notifies provider/nursing as appropriate.
    • Obtaining procedure/test reports for procedures performed at outside healthcare facilities.
    • Assists scheduling by communicating needs, confirm scheduling of referrals, etc.

Qualifications

Required as an LPN:

  • Current Licensure as a Licensed Practical Nurse (LPN) from the MN Board of Nursing
  • Documentation of completion of a practical nursing program (LPN program)
  • Current certification in CPR-BLS for Health Care Providers by the American Heart Association is required. If candidate holds CPR for healthcare providers through other acceptable organization (as determined by Gillette), candidate may be able to start employment but must complete CPR – BLS for Health Care Providers through American Heart Association within one month of hire.
  • Must be interested in working with people with short term or long-term disabilities that begin in childhood.
  • Physically able to work hours required and meet physical demands of the position.

Preferred

  • Prior nursing experience in a health-related setting
  • Specialty certification in an area of care related to position

Required as a MA/CMA/RMA:

  • Completion of an accredited Medical Assistant program.
  • Current Certified Medical Assistant (CMA) certification through the AAMA.
  • Current Registered Medical Assistant (RMA) registration through the AMT.
  • If non-CMA or RMA, must obtain Medical Assistant certification/registration by either the AAMA (CMA) or AMT (RMA) within 6 months from date of hire.
  • Current BLS certification (Basic Life Support through the American Heart Association) or obtain within 90-days of hire.

Preferred

  • Experience in a hospital or clinic environment.
