Job summary

Foundation Medical Partners, affiliated with Southern New Hampshire Medical Center, is seeking an LPN to provide patient-centered nursing care across in-person, telephone, and telehealth settings in Nashua, NH.

You will perform assessments, contribute to care plans with an interdisciplinary team, educate patients, and document care with accuracy while adhering to state Nurse Practice Acts and professional standards.

Qualifications

• Licensed Practical Nurse with current NH or compact state license.
• Current BLS/CPR training.
• Graduate from an accredited school of nursing or currently enrolled in an accredited program leading to licensure as an RN and has NH license as an LPN.

Responsibilities

• Participate in nursing assessments, interventions, and progress evaluation under RN and Provider supervision.
• Help design and implement an interdisciplinary plan of care.
• Educate patients on self-management, medications, nutrition, health and wellness as per plan.
• Document nursing processes and care in the medical record.
• Administer medications, treatments, vaccines, and perform point-of-care testing within scope.

About the Job

The Licensed Practical Nurse (LPN) provides patient-centered nursing care under the supervision of Registered Nurses, Providers, and the Practice Manager. This role supports clinical operations through patient assessment, care coordination, education, medication administration, documentation, and clinical procedures while collaborating with the healthcare team to deliver safe, high-quality care across in-person, telephone, and telehealth settings

What You’ll Do:

• Under supervision of the RN and Provider(s), participate in the nursing process, in-person, by telephone (telephone triage) and via telehealth modalities in accordance with organizational standards, state Nurse Practice Act, and other professional standards: performing nursing assessments, interventions, and evaluating progress toward defined goals of care.
• Participate in designing and implementing a plan of care with the interdisciplinary team including Registered Nurses, Physicians, Advance-Practice Nurses (APRN, CNM, CRNA’s), Physician Assistants, and other health professionals.
• Participate in educating patients on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).
• Document participation in the nursing process in the medical record with accuracy and detail.
• Administer prescribed medications, treatments, and vaccines; perform point-of-care testing; collect and handle specimens within scope of practice as determined by the Nurse Practice Act of the appropriate state.

Who You Are:
