5 West - Oasis Unit
5W Oasis Transitional Care Unit (OTCU) is a 14-bed inpatient unit providing care for patients with complex discharge barriers. Oasis has a diverse, interdisciplinary team that addresses these barriers in an innovative, holistic care environment. Quality treatment for patients with medical and psychosocial needs is provided by integrating nursing care and psychosocial assessment. The team facilitates meaningful patient activities such as music, art, and wellness programs. Each patient is treated with dignity, and their strengths are identified through highly personalized care. The Oasis team is high-performing, making a positive impact on patient outcomes with their innovative approach. This nursing team is highly engaged and ranks high in employee engagement surveys. The 5W Oasis nursing care model involves an RN/LPN/CNA partnership with nurse-to-patient ratios of 1:4 - 1:5 for RNs/LPNs and 1:7 for CNAs.
Job Summary
Under close supervision of an RN, Care Provider, or Nursing Clinical Coordinator, provides direct patient care according to established protocols. Assists RNs and Care Providers with procedures, medication administration, phlebotomy, and limited laboratory procedures. Assesses patients' physical and psychological conditions continuously and documents medical status throughout treatment.
Essential Functions:
  • Implements care based on patient needs under RN direction. (10%)
  • Obtains vital signs and reports abnormalities. (10%)
  • Administers medications safely. (10%)
  • Collects and processes laboratory specimens, blood samples, and manages IV access as ordered. (10%)
  • Performs specialized tasks such as EKGs, Foley insertions, spirometry, etc., as directed. (5%)
  • Documents Daily Goals/Plan of Care accurately. (5%)
  • Follows hospital principles of clinical practice and population health improvement. (5%)
  • Contributes clinical data to the Plan of Care with RN. (5%)
  • Performs chart audits for quality improvement. (5%)
  • Reports patient/family responses or changes to RN, updates care plans accordingly. (5%)
  • Participates in nursing research and practice changes. (5%)
  • Engages in outcome-focused care planning with measurable goals. (5%)
  • Coordinates care with multidisciplinary team. (5%)
  • Participates in discharge planning. (5%)
  • Assists in identifying realistic outcomes to optimize patient health. (5%)
  • Works with RN to develop personalized, outcome-focused care plans. (5%)

Education: Post High School Diploma or Certificate Program Required

Work Experience: No experience required

Licenses: BLS, IV Certification, and LPN license required

Knowledge, Skills, and Abilities: Interpersonal communication, assistance with personal hygiene, transport skills, IV monitoring, basic typing, charting knowledge preferred.

Shift: Varies (United States of America)

Work Type: PRN/Intermittent

Salary: $24.69 - $37.04/hr
