Job summary

Altra Home Health is seeking a compassionate Licensed Practical Nurse (LPN) to deliver in-home care to pediatric and adult patients in Noblesville, Indiana. You will perform comprehensive assessments, manage G-tube feedings, wound care, trach and ventilator support, and partner with physicians and therapists to create personalized care plans.

Shifts range from 4 to 12 hours with flexible day, evening, and weekend scheduling.

Qualifications

• Active LPN license in Indiana or an active compact LPN license.
• Minimum of 1–2 years of clinical nursing experience (home health and pediatric experience preferred).
• Experience with G-tube, trach, or vent care is a plus (training provided).
• Current CPR certification.
• Sharp critical thinking and confident independent decision-making.
• Outstanding interpersonal and communication skills.
• Reliable transportation and a valid driver's license.

Responsibilities

• Conduct head-to-toe assessments and implement personalized care plans.

Job Summary

Altra Home Health is looking for a caring and skilled Licensed Practical Nurse (LPN) to become part of our team. In this rewarding position, you'll deliver personalized, top-notch care to pediatric and adult patients in the comfort of their own homes. Many of our patients face complex medical challenges, including gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and occasionally tracheostomy or ventilator support. You'll be instrumental in fostering safety, comfort, and quality of life—empowering patients to receive the care they need right where they feel most comfortable: home.

Responsibilities

• Conduct comprehensive head-to-toe assessments and carry out personalized care plans.

• Deliver hands-on care such as G-tube feedings, medication administration, wound care, trach care, and ventilator support.

• Approach every patient interaction with professionalism and genuine empathy.

• Guide and empower patients and caregivers with education on disease management, emergency response, and therapeutic techniques.

• Partner closely with physicians, therapists, and fellow team members to deliver the best possible care.

• Keep accurate, timely documentation of all care activities in line with agency and state standards.

• Uphold the highest standards of safety, infection control, and patient privacy.
