Licensed Practical Nurse (LPN) - Post Acute Inpatient Rehab (UPMC Carlisle)

Position Details:

  • Full-time Regular, 36 hours per week, Night Shift
  • Work Schedule: 6:45pm to 6:45am
  • Every other weekend, every other holiday, no on-call
  • 20 bed unit
  • This position is eligible for a generous sign-on bonus

Purpose:

Act as a member of the healthcare team by exercising sound nursing judgment based on preparation, knowledge, experience, and competency. The LPN provides care and IV therapy following UPMC LPN Practice Guidelines, policies, and State Board Regulations. The LPN participates in planning, implementing, and evaluating nursing care using focused assessments under the supervision of a Registered Nurse or other licensed provider. The role requires taking nurse assignments at the top of the LPN's license in med/surg/telemetry or Emergency Department settings.

Responsibilities:

  • Exercise sound nursing judgment, participate in planning, implementation, and evaluation of patient care, and communicate effectively with the healthcare team. Seek guidance when patient needs exceed the LPN scope or knowledge, or if patient condition deteriorates. Follow established policies, procedures, and practice guidelines. Formulate daily goals and care plans involving the patient as a partner, considering individual needs holistically. Document comprehensively to promote communication.
  • Enhance patient safety, quality, and satisfaction by creating a caring, compassionate, patient-focused environment. Build healing relationships with patients, families, and colleagues. Participate in unit activities, provide patient and family education, support orientation and development of new staff, seek feedback, and pursue continuous professional growth. Identify opportunities for clinical and workplace improvements, collaborate with the healthcare team, and maintain productive relationships. Demonstrate cultural awareness and accountability, promote safe work practices, and communicate safety hazards.
