Base pay range

$35.00/hr - $50.00/hr

We seek a compassionate and skilled RN or LPN in the Paducah, KY area to provide patient care according to a physician’s care plan. This is a 1099 contract position, with care provided at the patient’s residence. Patients may require weekly visits or 24-hour care, offering flexible scheduling options, from short weekly visits to full 12-hour shifts or a combination.

Once accepted into our program, there is no expiration for this benefit, allowing caregivers to build lasting relationships with patients. You will advocate for patients and work closely with them, collaborating with CNAs, HHAs, case managers, and physicians.

Shifts are flexible, including night and weekend options, ideal for PRN or full-time work. Offers are contingent upon passing a background check.

Responsibilities
  • Execute physician-prescribed care plans
  • Document care provided accurately
  • Manage medications, treatments, and therapies
  • Conduct patient assessments and coordinate care
  • Assist with activities of daily living
  • Monitor vitals and GI intake/output
  • Assess skin integrity and perform wound care
  • Observe changes in patient condition
  • Communicate with family, physicians, and case managers
  • Provide education and training to patients and families
  • Work one-on-one with patients at home
  • Supervise CNAs and Personal Care Attendants
  • Perform other care duties as needed
Minimum Qualifications
  • Graduate of an accredited nursing program (Associate or Baccalaureate)
  • Current State or Compact RN or LPN license
  • CPR certification
  • TB test and physical exam
  • Maintain required insurance
  • Good interpersonal skills
  • Manual dexterity and hand-eye coordination
  • Ability to stand/walk for long periods, lift up to 50 lbs, and move patients
  • Ability to work under stress and make quick decisions
  • Dependable transportation and willingness to commute
  • Ability to provide basic to end-of-life medical management
Preferred Qualifications
  • At least 1 year of clinical or home health experience as RN or LPN
  • Ability to assist with two-person transfers
