Job summary

ClearSky Rehabilitation Hospital of LakeShore is seeking a Licensed Practical Nurse to join our night-shift team at our new Cleveland, WI facility. You will provide bedside nursing care under the guidance of a Registered Nurse and physician orders, ensuring adherence to care plans and hospital protocols.

Requirements include an active LPN license, two years of inpatient experience (rehab preferred), and current BLS.

Qualifications

• Must hold an active LPN license.
• Two years’ inpatient hospital experience; rehab experience preferred.
• Current BLS certification required.
• Knowledge of infection prevention standards and patient care protocols.

Responsibilities

• Provide skilled nursing care to patients under RN supervision and physician orders.
• Administer medications and treatments per physician orders.
• Contribute to development of individualized care plans and patient education.
• Identify patient risks and learning needs; participate in interventions.
• Maintain accurate documentation and hospital charging procedures.

This position must integrate company values into daily practice.This position will serve on the night-shift.Job Duties include, but are not limited to:Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.Implements orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the RN.Provides information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.Participates in evaluation of the effectiveness of interventions in relation to patient responses and seeks guidance in redirection when necessary.Helps identify patient problems and potential risks and contributes to the development of interventions.Assists in identification of patient’s learning needs and reinforces self-care and other techniques.Appropriately designates applicable charges to individual patients using the hospital’s established system.May be required to work during inclement weather and other staffing emergencies.Maintains regular attendance in keeping with assigned work schedule.Requirements for consideration:Two years’ experience in inpatient hospital setting, inpatient rehabilitation experience preferred.Current state license as a Licensed Practical Nurse required.Current BLS certification required.Knowledge of and adherence to current nursing theory and practice and infection prevention standards of practice.Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS.Physical Requirements Over the Course of a Shift:A significant amount of standing, walking, bending, reaching, lifting, pushing, and pulling, often for prolonged periods of time.Both gross and precise motor functions.Lifting/exerting of up to 50 lbs.Possible exposure to bodily fluids.Visual acuity required for patient assessment and documentation of care.Acute hearing required for accurate patient assessment.Sufficient manual dexterity to operate equipment and computer keyboard.Close vision and the ability to adjust focus.We value our employee’s skills, talents and input.
