Job summary

Plaza Community Living Center is seeking a licensed practical nurse (LPN) for full-time night shifts (7pm–7am) to deliver high-quality resident care and support. The role involves medication administration, care planning, rounds, and comprehensive documentation.

Qualifications

• LPN license in good standing is required.
• BLS certification required.

Responsibilities

• Evaluate and deliver care including medication administration, treatments and ADL support as ordered by physician and plan of care.
• Conduct regular rounds to monitor resident activity and ensure quality care.
• Communicate with residents, family members, physicians and supervisor regarding resident status.
• Maintain up-to-date medical records during tour of duty on assigned unit.

Licensed Practical Nurse ( LPN ) duties include but are not limited to:

• Evaluation and delivery of care including medication administration, treatments, Adl care, etc. as ordered by physician and outlined in the plan of care (according to scope of practice)
• Conduct regular rounds to monitor resident activity and ensure resident/patients receive quality care based on professional standards and facility policies
• Communicate with residents, family members, physicians and supervisor regarding resident status
• Maintain up to date and accurate medical records during tour of duty on assigned unit.
• Other tasks as required
