Centerville Place Assisted Living in Dayton, OH is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care and supervise CNAs in accordance with regulations and facility policies.

Responsibilities include directing nursing staff, promoting holistic care, administering medications, handling admissions/discharges, and coordinating follow-ups. LPN license, CPR, and IV certification are preferred; nursing degree required.

Qualifications

• Long-term care experience preferred but not required.
• Current CPR certification.
• IV certification, if applicable.
• Nursing degree from an accredited college or university.

Responsabilités

• Directs daily activities of Certified Nursing Assistants in line with regulations and policies.
• Promotes holistic care following the nursing model.
• Ensures policy compliance by nursing staff.
• Participates in admissions, discharges, and transfers as required.
• Assess changes in residents' status and notify physician and family; document.
• Transcribes physician orders and carries them out as written.
• Prepare and administer medications per physicians' orders; observe adverse effects.
• Perform wound care per orders and document changes.
• Schedule follow-up appointments and transportation as needed.
• Provide nursing leadership to the unit staff.
• Other duties as assigned.

Position Summary

The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures.

Key Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Follows the nursing model to promote holistic care for the residents.
• Ensures that policies and procedures are complied with by nursing personnel assigned.
• Participates in the admission, discharge and transfers of residents as required.
• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.
• Transcribes physician orders to medical record and carries out orders as written.
• Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated.
• Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated.
• Schedules follow up appointments for residents and transportation needs as indicated.
• Provides nursing leadership to nursing personnel assigned to the unit.
• Other duties as assigned.

Qualifications

• Prior experience in long-term care preferred but not required.
• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.
• IV certification, if applicable
