Position Summary: The Licensed Practical Nurse performs assessments and simple medical nursing procedures in clients' homes under the guidance of the RN Supervisor. They are responsible for implementing nursing care based on the plan developed by an RN, in accordance with physician's orders.

Location: Irvington, AL 36544

Hours: Monday and Tuesday night, 8 pm to 6 am

Essential Duties

• Perform nursing procedures within the scope of LPN practice, such as tracheostomy care, colostomy care, enemas, douches, catheterizations, dressing changes, and phlebotomies.
• Provide patient and family teaching as outlined by the RN.
• Monitor vital signs and record measurements and observations.
• Monitor and record intake and output, reporting significant changes immediately.
• Assess and document patient's physical and mental condition, reporting any changes.
• Administer medications as instructed by the physician and RN, documenting appropriately.
• Coordinate patient care with patients, families, and healthcare team members.
• Maintain confidentiality of sensitive information.
• Maintain regular attendance and be flexible to meet the agency's needs.
• Follow all Medicare, Medicaid, and HIPAA regulations.
• Adhere to all policies, procedures, and standards.
• Perform other duties as assigned.

Position Requirements & Competencies

• Graduate from an accredited practical nursing program.
• Licensed as a vocational/practical nurse in the state of practice.
• At least two years of clinical practical nursing experience.
• Ability to make home visits and provide nursing care in the home environment.
• Ability to complete documentation accurately and timely.
• Strong interpersonal, verbal, and written communication skills.
• No weight restrictions.
• Reliable transportation, current driver's license, and appropriate insurance, or ability to use public transportation.
