• Complete required record keeping related to admitting, transferring, and discharging residents.
• Coordinate daily with nursing staff and support personnel to plan shifts’ services, programs, and activities, ensuring the resident’s comprehensive care regimen is maintained.
• Prepare, administer, and review medications and medication cards for accuracy and completeness.
• Review and update resident care plans to reflect appropriate goals, problems, approaches, and revisions based on nursing needs.
• Develop, implement, and interpret programs, goals, policies, and procedures necessary for quality care.
• Provide leadership to nursing staff, make daily rounds to ensure care aligns with resident care plans and wishes.
• Build rapport with nursing staff, other departments, residents, families, and all involved in resident care.
• Participate in facility surveys by authorized government agencies.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must hold a current, unencumbered, active LPN license in this state.
• Knowledge of nursing and medical practices, laws, regulations, and guidelines pertaining to nursing care facilities.
• Ability to chart nurses’ notes informatively and descriptively, reflecting care provided and resident responses.
• Experience with Electronic Medical Records and computer documentation systems.
• Maintain confidentiality of all resident care information per HIPAA guidelines.
• Fluent in speaking, reading, and writing English.
• Active CPR/BLS certification.
