- Complete required record keeping related to admitting, transferring, and discharging residents.
- Coordinate daily with nursing staff and support personnel to plan shifts’ services, programs, and activities, ensuring the resident’s comprehensive care regimen is maintained.
- Prepare, administer, and review medications and medication cards for accuracy and completeness.
- Review and update resident care plans to reflect appropriate goals, problems, approaches, and revisions based on nursing needs.
- Develop, implement, and interpret programs, goals, policies, and procedures necessary for quality care.
- Provide leadership to nursing staff, make daily rounds to ensure care aligns with resident care plans and wishes.
- Build rapport with nursing staff, other departments, residents, families, and all involved in resident care.
- Participate in facility surveys by authorized government agencies.
Qualifications
- Must possess a nursing degree from an accredited college or university.
- Must hold a current, unencumbered, active LPN license in this state.
- Knowledge of nursing and medical practices, laws, regulations, and guidelines pertaining to nursing care facilities.
- Ability to chart nurses’ notes informatively and descriptively, reflecting care provided and resident responses.
- Experience with Electronic Medical Records and computer documentation systems.
- Maintain confidentiality of all resident care information per HIPAA guidelines.
- Fluent in speaking, reading, and writing English.
- Active CPR/BLS certification.
