Position Overview:

The ideal Licensed Practical Nurse (LPN) will have a strong background in patient care, medical documentation, and health coaching. This role involves working closely with patients in long-term care (LTC), memory care, and outpatient facilities.

Key Responsibilities:

  • Provide direct patient care under the supervision of RNs or physicians
  • Conduct health coaching sessions to promote wellness and chronic condition management
  • Perform accurate medical documentation in compliance with healthcare regulations
  • Assist in outpatient settings, LTC, and hospital medicine environments
  • Administer medications and treatments as prescribed
  • Monitor patients’ vital signs and report any changes
  • Support patients undergoing dialysis and educate them about the process
  • Utilize medical terminology to communicate effectively with healthcare teams
  • Conduct diagnostic evaluations to assess patients’ health status
  • Ensure infection control protocols and maintain a safe patient environment

Requirements:

  • Active LPN License in the state of practice (VT, NH, or compact license)
  • Minimum 2 years of Long-Term Care (LTC) experience
  • Must be able to adapt quickly and work with minimal orientation
  • Experience working in memory care settings preferred
  • Excellent communication skills – verbal and written
  • Ability to work collaboratively while maintaining patient confidentiality
  • Flexibility to adapt to varied healthcare environments and schedules
