Job summary

Care Advantage, Inc. is seeking multiple Licensed Practical Nurses (LPNs) to support our High Acuity Division across Virginia. We provide one-on-one home health care, developing specialized plans with Registered Nurses to keep patients safe and comfortable at home.

Qualifications

• Must possess an active LPN license in the United States.
• 1+ year experience as an LPN preferred.

Responsibilities

• Provide and implement direct patient care in clients' homes.
• Provide accurate and timely documentation per plan of care.
• Educate family/caregivers and patients on care and self-care.
• Assist RN or physician with procedures, prepare equipment, and maintain aseptic technique.
• Coordinate home health services and report needs to the RN/Clinical Supervisor.

LPN Responsibilities:

• Provide and implement direct patient care
• Provide accurate and timely documentation consistent with the plan of care
• Assess and provide family/caregiver education and information
• Assist the RN or physician in performing specialized procedures and duties, which include preparing equipment and materials for treatments, observing aseptic technique as required, and assisting the patient in learning appropriate self-care techniques
• Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor

Requirement/Eligibility:

• Eligible to work in the United States
• CPR Certification and PPD Certification
• 1+ year experience as an LPN
• Valid LPN License with at least (1) one year of work experience
• Valid Driver's license
• Reliable transportation
