UC Health is hiring a part-time License Practical Nurse (LPN Meds) for Bridgeway Pointe for the varying shifts at Daniel Drake Center for Post-Acute Care.

The Bridgeway Pointe Nurse will support Daniel Drake’s Bridgeway Pointe in delivering high-quality care to patients based on assessed needs, established standards of care, and according to policy and procedures. This unit has a total of 72 residents within our assisted living facility and additional beds for memory care. Our nurses help pass medications to these residents and provide the best care within our facility.

Daniel Drake Center is the region's premier provider of post-acute and transitional care services, offering inpatient and outpatient services including complex care, skilled nursing, assisted living, wellness services, and research. It helps patients recover from injuries and illnesses such as multi-system organ failure, complex wounds, traumatic brain injury, stroke, and spinal cord injury.

As the only hospital-based skilled nursing facility in Cincinnati, patients have access to services like 24/7 on-site physician coverage, pharmacy, diagnostic imaging, wound care, and specialty physician consulting. These on-site services enable quick responses to complex care needs.

Qualifications

Job Responsibilities

The LPN Meds holds a current state license to practice nursing. The role requires specialized knowledge, judgment, and skills to care for individuals and groups. The LPN Meds applies principles from biological, physical, behavioral, social, and nursing sciences to assess, plan, implement, and evaluate patient care, which is culturally and age-specific.

  • Contribute recommendations to the RN regarding the interdisciplinary care plan for each patient, collaborating with the healthcare team within the unit's standards.
  • Assist the RN in establishing and prioritizing patient problems and needs.
  • Communicate care plans effectively to team members and prioritize care based on patient assessments.
  • Provide clear instructions to patients and families, involving them in treatment and ongoing care.
  • Identify and respond appropriately to behavioral health, psychological, or cultural needs.
  • Demonstrate critical thinking, accountability, flexibility, and teamwork.
  • Accurately document nursing care and patient responses, following hospital policies.
  • Serve as a resource and mentor for new and temporary staff.
  • Engage in ongoing professional development and support peers' growth.
