STGi is seeking a LPN/LVN to provide services at our Georgia War Veterans Home. GWVH will be opening a new unit dedicated to Veterans with PTSD, TBI, behavioral disorders, and substance use issues. If you are compassionate and passionate about caring for Veterans, this could be the job for you!

Job Summary

The primary purpose of the LPN/LVN is to provide direct nursing care to residents.

Essential Functions
  1. Ensure staffing levels meet requirements and complete necessary postings.
  2. Assess residents experiencing significant changes using the Resident Assessment Instrument (RAI).
  3. Assist in developing dementia care programs with an interdisciplinary approach.
  4. Verify resident census accuracy and document admissions, discharges, and bed-holds.
  5. Assist with resident transfers and discharges safely and in accordance with policies.
  6. Coordinate pre-certification and recertification for insurance payers.
  7. Interpret policies and procedures to staff, residents, visitors, and agencies.
  8. Participate in developing and updating facility policies and procedures.
  9. Participate in facility surveys and audits for documentation and coding accuracy.
  10. Perform administrative duties like charting, reports, and evaluations.
  11. Review policies, job descriptions, and make recommendations for revisions.
  12. Coordinate resident billing and provide nursing care as per care plans.
  13. Communicate with physicians and care teams regarding resident status.
  14. Arrange translation services for LEP residents.
  15. Obtain orders and ensure residents are treated with dignity and respect.
  16. Provide culturally competent, trauma-informed care.
  17. Evaluate residents’ responses to care and coordinate person-centered plans.
  18. Ensure residents are free from unnecessary restraints and coordinate nursing services with other departments.
  19. Identify educational needs and provide resident and family education.
  20. Monitor social interaction and signs of distress, and implement dementia care approaches.
  21. Assist with restorative programs and pain management.
  22. Complete incident reports and coordinate nutritional and therapeutic services.
  23. Keep management informed and cooperate with social and activity programs.
  24. Review discharge plans and ensure safe discharges with proper documentation.
  25. Supervise CNAs and ensure proper medication administration and safety protocols.
  26. Support infection control and safety procedures, participate in drills, and ensure equipment safety.
  27. Manage supplies, equipment repairs, and coordinate with staff for effective shift services.

Required Experience

  • Minimum one year supervisory experience in a healthcare setting.
  • Training in rehabilitative and restorative nursing practices.
  • Current, active nursing license in Georgia.
  • Proficiency in English and ability to provide age-appropriate care.

Note: Selected candidates will receive safety care training during orientation. This position is available for all shifts at 2249 Vinson Highway SE, Milledgeville, GA.
