Altra Home Health is seeking a Licensed Practical Nurse (LPN) to deliver in-home care for pediatric and adult patients in La Porte, Indiana. You will perform comprehensive assessments, administer medications and manage G-tube, trach, and ventilator care as needed, ensuring safety and quality at home.

You will educate patients and caregivers, collaborate with physicians and therapists, and maintain thorough documentation while upholding safety and infection-control standards.

Qualifications

• Active LPN license in Indiana or an active compact LPN license.
• Minimum of 1–2 years of clinical nursing experience (home health and pediatric experience strongly preferred).
• Experience with G-tube, trach, or vent care is a plus (training provided).
• Current CPR certification.
• Sharp critical thinking and confident independent decision-making.
• Outstanding interpersonal and communication skills.
• Reliable transportation and a valid driver's license.

Job Summary

Altra Home Health is looking for a caring and skilled Licensed Practical Nurse (LPN) to become part of our team. In this rewarding position, you'll deliver personalized, top-notch care to pediatric and adult patients in the comfort of their own homes. Many of our patients face complex medical challenges, including gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and occasionally tracheostomy or ventilator support. You'll be instrumental in fostering safety, comfort, and quality of life—empowering patients to receive the care they need right where they feel most comfortable: home.

Responsibilities

• Conduct comprehensive head-to-toe assessments and carry out personalized care plans.

• Deliver hands-on care such as G-tube feedings, medication administration, wound care, trach care, and ventilator support.
