Qualifications

Job description

Maxim Healthcare in Denver is hiring for a Licensed Practical Nurse (LPN) to work one-on-one with patients needing intermittent homecare visits. Most common type of cares needed include catheter care, wound care, diabetic management, and medication management. Visits range from 30 minutes- 1 hour in duration.

Responsibilities:
  • Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
  • Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
  • Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
  • Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
  • Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
  • Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable
  • Supports professional practice by observing the following:
    • Nurse Practice Act
    • Standards of Nursing Practice
    • Standards of Patient Care
    • Standards of Accrediting Body (ACHC, Joint Commission, etc.)
    • State and Local Laws
    • Patient Bill of Rights
    • Maxim Healthcare, Nursing, and Client’s Policies
  • Adheres to Maxim policy and procedure and applicable protocols
  • Attends case conferences, team meetings, education in-services as required
  • Performs other duties as assigned

Requirements:
  • Graduate of an accredited school of nursing
  • Current Licensed Practical Nurse (LPN) License for the state of Colorado.
  • Current PPD or Chest X-Ray.
  • Current BLS certification.
  • One year prior Licensed Practical Nurse (LPN) experience preferred, but not required.
  • Complies with accepted professional standards and practice
Salary:
*Pay structure is per visit
  • $35-40/visit
