Job summary

BAYADA Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in the North Jersey Hospice office, North Bergen, NJ for per diem visits. This role covers hospice clients in North Jersey with weekly pay and requires reliable transportation.

Minimum 2 weekdays availability, 1-2 holidays per year, no on-call, and ability to travel to assigned counties. Bilingual Spanish a plus; base pay $38–$41 per hour depending on qualifications.

Qualifications

• Current Licensed Practical Nurse in NJ.
• One year of clinical experience preferred.
• Home care or hospice experience preferred but not required.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, provide wound care, treatments, and comfort care.
• Monitor client conditions and report changes to supervisor.
• Follow up on doctors' orders and document them.
• Perform client assessments as needed.
• Occasional on-call after hours/weekends.

License Practical Nurse for Hospice Visits

BAYADA Hospice is looking for compassionate and dedicated Licensed Practical Nurse, LPN to join our team in the North Jersey Hospice office, located in North Bergen, NJ for per diem visits. This office services hospice clients on a per visit basis in North Jersey (Primarily Essex, Union, Middlesex counties).

• Minimum of 2 weekdays availability required
• 1-2 holidays per year mandatory
• No on call required
• Must be willing to cover any county-assignments will vary
• Weekly pay
• Bilingual Spanish a plus
• This is a visiting position-reliable transportation needed
• Base Pay, depending upon qualifications and subject to negotiation: $38-$41
• LPNs must be comfortable with Hospice admissions

Prior home care and/or hospice experience strongly preferred, but not required. As a hospice nurse you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a Hospice LPN include

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Occasional on call after hours and/or weekends

Qualifications for a LPN include:

• A current license as a Licensed Practical Nurse in NJ
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care and/or hospice experience strongly preferred, but not required.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
